It isn't strictly required that the red square be outside, but if it is inside another confined room instead, that room will become very hot very quickly and can become dangerously hot for your colonists, even without a heatwave, and a heatwave can make that situation much worse. Air conditioning is the most obvious method to fighting heat waves, and is pretty simple: plop down a power source, and place an AC unit on an outer wall of a structure, rotating it so that the blue square is on the inside of the structure (this indicated the side that will be cooled), and the red square is on the outside of the structure.
